Elden Ring: Millicent Quest, Prosthesis, and Final Choice

Millicent’s quest crosses Caelid, Altus, the Mountaintops, and Elphael. Its final summon signs determine which talisman you receive, so decide what you want before selecting either one.

Repair the first needle in Caelid

Start by speaking to Gowry in his shack south of Sellia; he tells you the needle is needed. Defeat Commander O’Neil in the Swamp of Aeonia for the Unalloyed Gold Needle, hand it to Gowry, reload the shack, and speak to him again for the repaired needle. At Church of the Plague, give it to Millicent, rest, exhaust her dialogue, then return to Gowry’s Shack and exhaust Millicent’s new conversation there.

She later moves through Altus. Find her north of Erdtree-Gazing Hill, then take the Valkyrie’s Prosthesis from the northwest Shaded Castle chest. A Cleanrot Knight guards that chest. Give the prosthesis to Millicent, then defeat Dominula’s Godskin Apostle and speak to her by Windmill Heights until her dialogue ends.

Keep checking the later positions

Millicent next waits near Ancient Snow Valley Ruins in the Mountaintops and then at Elphael’s Prayer Room. To reach Elphael, obtain the right Haligtree Secret Medallion from Albus disguised as a pot in the Village of the Albinaurics, defeat Commander Niall at Castle Sol for the left half, use the alternate lift action at Grand Lift of Rold, light Ordina’s four Evergaol flames, then take the released portal. Defeat Loretta to enter Elphael and reach Prayer Room.

From Prayer Room, continue through Elphael to Inner Wall and then Drainage Channel. From the grace, go back uphill toward the rot river, take the right-hand rise and cross the root bridge to the separate Scarlet Rot pool. Drop down to defeat the Ulcerated Tree Spirit, then return to the pool’s upper ledge to find the two summon signs. Do not continue forward toward the final church.

Make the final choice deliberately

The gold sign sends you to help Millicent fight four sisters. Success gives Rotten Winged Sword Insignia; speak to Millicent afterward and reload to recover the Unalloyed Gold Needle. The red sign sends you to kill Millicent with those sisters and rewards Millicent’s Prosthesis instead.

These are different quest endings and reward paths. Do not click whichever sign happens to be closest. If you help Millicent, the recovered needle can later become Miquella’s Needle after Malenia is defeated and her Scarlet Aeonia is examined. Follow the Frenzied Flame reversal route for the needle’s final use in Placidusax’s arena.
