Hand fighters break into the hospital to reclaim the blood-draining victims. Claire's colleagues are killed during the attack, and Matt arrives to stop her falling to her death. The captives leave with the Hand rather than accepting the rescue he thought he had provided. Afterwards, the hospital administration wants the incident concealed despite evidence that one dead attacker had already undergone an autopsy. Claire refuses to participate in the cover-up and resigns, leaving Foggy as he prepares to go home.
Karen insists Frank did not murder Reyes and meets him secretly despite the police assigned to protect her. At a diner, their conversation about the Blacksmith and Matt turns violent when armed men arrive. Frank uses Karen as bait for the attackers, forces a location from one of them and kills him. Matt pursues his own lead through Madame Gao, who gives him information about the drug operation rather than pretending the Blacksmith poses no threat to her.
Matt and Frank meet at the pier, where Frank believes he has found the man responsible. Matt identifies a deception, but more gunmen attack before he can resolve it. Frank gets Matt away from the boat as it explodes, leaving Karen and the police to confront the wreckage. Elsewhere, Elektra goes after Stick for sending an assassin. A dying member of Stick's group reaches Matt with a warning, while the former teacher waits for Elektra to arrive.
