Sawyer plans to electrocute a guard using his cage's wiring, but Ben overhears him through surveillance and cuts the power. After beating and restraining Sawyer, the Others stage a procedure and tell him a pacemaker will kill him if his heart races. Ben also threatens to give Kate one if Sawyer reveals it. Believing the warning, Sawyer does not fight back when Pickett attacks him. Afterward, he refuses Kate's attempt to free him.
Flashbacks show Sawyer cultivating Munson, a prisoner who has hidden stolen money. He persuades Munson that the warden is using his wife to find the stash, eventually gaining its location himself. Sawyer then gives that information to the warden in exchange for a reduced sentence and a reward. Although he rejects Cassidy's claim that her baby is his daughter during a visit, he directs the money into an account for the child, Clementine.
Juliet brings Jack to treat Colleen, but she dies despite his efforts. Jack notices unrelated spinal X-rays and realizes that another patient has a large tumor. At the beach, Desmond builds a lightning rod that is struck during a storm beside Claire's shelter. Finally, Ben takes Sawyer uphill and admits the pacemaker was a bluff. He shows him the sedated rabbit used in the deception and the larger island across the water, revealing that the cages are on a separate island.
