Michael knocks Locke unconscious, takes a rifle and locks him in the gun room with Jack before leaving to search for Walt. Kate and Sawyer release them, and Jack heads into the jungle with Locke and Sawyer. Jack refuses to let Kate join, but she follows anyway. The men find spent ammunition and continue despite disagreement over whether they should turn back. At night, the bearded man who took Walt confronts them and demands that they surrender their weapons. Jack challenges his claim to control the island until torches illuminate a surrounding group. The man then produces Kate as a hostage, forcing Jack to reconsider his refusal.
The men give up their guns in exchange for Kate's release and return to camp without Michael. Jack is angry at Kate for following them, while Sawyer reassures her that he would have done the same. Jack subsequently asks Ana Lucia how long it would take to train an army. Flashbacks show Jack attempting difficult surgery on a man whose daughter, Gabriela, has sought his help. Christian warns him about becoming emotionally involved. The patient dies, and Jack kisses Gabriela while trying to comfort her before pulling away. He confesses the kiss to Sarah and promises to improve their marriage. Sarah reveals that she has already met someone else and packed to leave, telling him that he always needs something to fix.
