Flashbacks reveal Claire's pregnancy and Thomas's initial promise to help raise their child. He later leaves, and Claire considers adoption. Psychic Richard Malkin repeatedly insists that she must raise the baby herself, then changes his advice and supplies money and a ticket to Los Angeles. He says a suitable couple is waiting there. Claire accepts the arrangement despite its contradiction, taking the flight after failing to complete another adoption agreement she had planned to sign.
On the island, Claire wakes from disturbing dreams and later says someone held her down and threatened her baby. Jack suspects stress and sleepwalking, offering a sedative that she sees as evidence he does not believe her. Hurley begins a census to establish who is living among them and obtains the flight manifest from Sawyer. Upset by Jack's response, Claire leaves the caves with Charlie, then experiences contractions while they are in the jungle.
Charlie asks Ethan to fetch Jack before returning to Claire. Hearing about Malkin, he suggests the psychic may have known the flight would crash and intended Claire to keep the child; it remains their interpretation rather than a confirmed explanation. Sayid returns to camp with a warning about other inhabitants. Hurley discovers that Ethan's name is absent from the manifest. In the jungle, Claire's contractions ease, but Ethan appears before her and Charlie, confronting them away from camp.
